Question on driving licence application form

On the driving licence appliance form, top of 2nd page question 12, it asks if you generally suffer from any disease, condition, disorder or addiction.
Has anyone ticked the answer?

I see on the application form that particular section tells people to See Note 14.

Translated from Greek Note 14 states:
If you have any doubts about your health status consult your GP or opthalmologist before completing the application form.

That sound a bit like the UK form where you just put a tick in a box to self-certify that you are fit to drive

Maybe its for things like epilepsy, were there are restrictions on driving ?
