I suppose human error is still one of the major causes leading to accidents, be it on aircraft or hearses!
I hope that the deceased’s relatives weren’t following the hearse at the time as seeing the coffin fall out would have been traumatic for them. It probably was bad enough for other drivers who witnessed this happening..
