MUKW3 biometric card application form

Oh no, not this again I hear you groan but would some kind person please supply me with the above application form and confirmation that as i have yellow slip permanent residency I only take that to immigration with passport and form. I'm really sorry about this as I know it has been discussed a lot but I just can't find the relevant information. I have been looking hours in total and wouldn't ask normally without looking first but I am chasing dead ends.

from MEU3 to new card you need originals and copies of passport and MEU3 & €30 cash.

Polly,dont panick,you have until 23rd August 2026 to get the coveted biometric card.

I updated to the MUKW3 last year. As I am traveling within EU countries shortly, this form which is in fact a biometric card which is credit card sized will fit in my wallet and be easily accessible when going through immigration at airports.
So it really supercedes the old MEU paper one.

Thank you all for your comments and thank you very much Jimgym for supplying link. I have had trouble printing but think I now have it safely in hand. I am reminded that I did find this some time ago and couldn't open it.

Some folk also say marriage certificate, so I’d take that along too with a copy

Pam and I attended immigration this morning for our 10.00 appointment. We had the completed application form for husband and spouse, original plus copies of MEU3s, original and copies of Passports and even copies of our Passports from when we had applied for our MEU3s back in 2018.

What could go wrong??

Although the appointment was in fact on time and the lady was very pleasant she insisted that each of us had to have a completed application form - one as me being the lead applicant and for Pam being the lead applicant.

The lady did give us a blank form to complete as showing Pam as the lead applicant but suggest anyone else applying make out a separate form for both husband and wife before going to Immigration.

Will someone please supply the information required on the attachment to send with the email asking for an appointment. I did have it but again I have lost it and can't find it anywhere. Sorry about this. It hasn't helped having had my laptop and phone stolen where this info was stored. Thank you very much.

Attached are my details as requested.
Sign full name.

In a word document
Heading (what you want the appointment for)
Name(s). Surname
Date of Birth.
Nationality
Passport number.
ARC number (MEU)
Date of last arrival in Cyprus (literally the last time you flew into a Cypriot airport)
Telephone number
Expiration date of residence permit (for an MEU = None)
File number (shown on MEU)
Number of Applicants

Attach the word document to the email and send. You will get a reply within 5 days
