NT tax code.

I have to get an NT( No Tax declaration from the UK)form, signed and stamped at the Paphos Tax Office ( Can't wait to go there!)....
So has anyone else done this recently?
What else do I need to take?
Where do I go?
I assume that at the mo its not possible to visit the tax office?
Any help appreciated.....ta
