Can anybody translate these?

I know Form A is for businesses and form B is for personal use, but other than that, I am not sure.

Where can they be collected from?

How many should I need?

Is it 1 for going to pharmacy, and another 1 for supermarket shopping?

News update now regarding the forms to move
You must complete the form A once for work reasons.
Form B to be completed every time you go out.
OR
You can send a text to 8998
For the text you must add the following answers with a space between each answer:
The reason why you are going out (the number relating to your reason 1-8, see below)
your ID/passport number
your post code
You will get a message back if you are allowed to go out.
The allowance is 3 hours once you get your approval only.
This permission is per person. If you will go out with another person they must send a text message for permission.
If a car has more than 2 people in it they must have a very good reason.
You should not be going to each other’s houses for coffee etc
The main message is to stay at home.
You are only allowed to move around in your local area.

I understand I must complete form B whenever I need to go to the supermarket, pharmacy etc. What isn't clear is what we do with it when completed. Do we hold onto it? if so, then we don't need further permission to leave the house, whereas if we send a text message we do.

supposing a person doesn’t have a printer or scanner what then? what do you do with the form if you take it with you? That isn’t clear.

Sorry memory man,but my hi tech knowledge is at zero.I have looked at Philinews but its all in greek and I cant find a form,,can you be more specific for and old timer please :?

I personally think that there will be very few people carrying these forms with them.. I wonder if the police, who may stop you will have them ?

You will get fined €150 if you haven't got a form or the text response, so I think people will have them. I saw somebody in Polemi with one just now.

There is no English version. We have added subtitles, but I would suggest filling in in Greek only one. Alternatively send a text, as explained.

You don't need a form if you send a text, but make sure you have your id and the phone showing the response to your text.
