Over 70 driving licence

Can anyone let me know which forms to download to renew driving licence for over 70s. I have downloaded form TOM 153E which I think is the medical form, and I have downloaded form TOM 7D, which I thought was to renew licence but not sure. Its 4 pages, but when I last renewed it was only a single page last time. :) :)
Thanks for any help you may be able to supply.

Norman as you only 'think' you have downloaded the Medical form, and you don't mention if it is a Cypriot licence or a UK one you are changing, would it not be better to get the correct forms from the Citizens Bureau ...

Agree with Trev. Pop along to the CSC and ask them for the correct forms.

Just renewed mine:
TOM:153E filled in by your doctor
TOM 7D
Original yellow slip
and of course your Cypriot driving licence that you want to renew

They filled in the TOM 7D for me.

Thanks Louise, seems I have downloaded correct forms. The main thing is we can have our medical and have that form completed by doctor before attending citizens advice to get other form completed.Trev it is Cypriot licence'

A renewal of a Cypriot licence will be less of a problem.. Some people use the doctors at the General to fill the form. Others like myself use a private doctor. Apparently, it is up to the doctor as to whether to sign for 1,2,or 3 years

That they say an over 70's licence is Normally issued for 3 years.

It would appear from previous comments that the Dr in the Gen Hosp. is the one that only signs for 1 year. Our private GP is always for 3 years as long as we are ably fit to drive.
