S 1 Form
Re: S 1 Form
As far as I can remember Paul we took our S1 forms to our GP, who passed them on with out application to join Gesy. We did take a copy to keep at home

Re: S 1 Form
Paul, did you receive it automatically after applying for your state pension or did you need to apply for it separately?
Thanks
Thanks

Re: S 1 Form
A while ago now.
Took S1 to Citizens Service Centre. White medical card sent in post. Valid for 3 years. Before that one expired a new one was sent. No sure if you still need to make an appointment? We did not need to as this was prior to covid.
On signing up with a Gesy Doctor took S1, white medical card and passport.

Happy to be back on the Island!
Re: S 1 Form
Here's what I have done.
I went to the citizens advice center with the S1 forms.
I filled in part of a form and the young lady said they will do the rest.
She took copies of my passport and my MEU3.
She asked if I had a copy of the S1 and I said yes I had scanned it on the computer to witch she said bravo.
I was told in about 3 or 4 weeks I will get a white card.
15 min's all done including waiting time.
